Community and Home-Based Services
Although Stevens is historically known for specialized residential programs, to assist youth and their families in making transitions from our residential programs Stevens has available Follow Along services which include short-term community and home-based support. This service is approved by the Department of Children and Families, and the Department of Mental Health. Where appropriate and recommended, Stevens’ staff provide this service during periods of transition. For youth transitioning from our campus-based residential programs, Follow Along Services include community and home-based interventions and supports to youth and families, post-discharge. The team providing services consists of experienced Master’s level licensed clinicians and direct care staff from Stevens that are integrated in the residential setting and continue to work with youth and families after the transition to a home setting. To support the transition after discharge, where appropriate Stevens provides access to selected treatment groups, and recreation activities.

At Stevens, we believe that partnership with youth and families is an essential component of quality treatment and meaningful organizational development. We encourage parents and families to engage with Stevens in the following ways:

Maintain communication with the Family and Community Engagement Coordinator |
Our Family and Community Engagement Coordinator is here to support the families through their time at Stevens. As part of the Admissions Office, the Family and Community Engagement Coordinator works with parents and guardians during the admissions process to ensure that families have the information they need. Families may experience obstacles to participation in services, and the Family and Community Engagement Coordinator helps families remediate those barriers. As youth prepare to transition from Stevens, this individual continues to provide guidance with the preparation for the transition to home and community. Post-discharge, the Family and Community Engagement Coordinator maintains connection with youth and families , and completes ongoing outcomes surveys.
